Foreign Travel

We know how important it is to be prepared and protected when traveling abroad. Jeff Chambers PA-C has participated and led many medical mission trips to Haiti through Bethlehem Ministries.

We have had many calls on what immunizations and prescriptions are needed to travel abroad. We are now offering a Travel Clinic consultation for your medical needs prior to traveling.


Please download and fill out our Foreign Travel Consultation form before your appointment and bring it with you.

Immunization Records

Please bring your immunization records with you. We can print out your records if in the State of Georgia database.


Please call our clinic to schedule your appointment. We recommend 2 to 3 weeks in advance or more before traveling.

  • Consultation fees include prescriptions, immunization consultation, and plan.
  • Does not include physical exam, blood work, or x-rays.
  • Individual consultation at $50.
  • Family consultation for 3 or more at $125.
  • Some insurance companies may cover these charges, but not all. This is part of the consultation.
  • Fees are subject to change due to changes in vaccine costs.
  • Self-pay for immunizations fees.
